Display mysql fetched data to an existing html form

I have an HTML form, and i have fetched data from mysql to php, i want to show those fetched data to that existing HTML form.

The form contains

• textbox


• date


• selectbox


• radio


• file


• textarea

Can any one help me to solve this problem
